New Patients

What to Expect

On your first visit, you will fill out our registration paperwork. You can download and print it from this website or you can have our staff email it to you. You can also fill it out when you come to our office but we ask that you arrive about 10 minutes early to complete it.

  • We will perform a thorough history and help to identify all your painful areas as well as difficulties your are having with your daily activities.
  • We will perform a comprehensive exam to help determine what tissues and systems might be involved in your case.
  • We will discuss our findings with you as well as recommendations for treatment or further testing or imaging.
  • If indicated we will begin some basic treatment and educate you on ways to improve your condition. This can include things like avoiding contributing activities and self-care strategies.

Payment Options

We try to make care affordable for everyone. A majority of our patients pay directly out of pocket for our services. We accept cash, check, credit card, Health Savings Account cards and some insurance.
Our typical fees:
Exams: Focused $45, Expanded $75, Complex $125
Office visits are typically in the range of $40-$60 and will include manipulation/mobilization of joints and soft tissues as well as exercise/home care instructions.


We are a participating provider for the following insurance companies.

  • Humana
  • Medicare
  • Medical Mutual
  • United Healthcare
You will need to check with your insurance to determine your individual level of coverage.
We can also work with your Personal Injury and Workers Compensation claims as well.
